Getting started is easy. For your first appointment, just call the ChiroWerx office at (980) 201-9484. Our team will work with you to get you on the schedule as soon as possible. After your first appointment, you can then book follow-up appointments in the office, call our office number, or schedule them online at your convenience.

Before You Arrive

Before your first chiropractic visit, please take a few minutes to complete the new patient intake form on our website (click the “Get in Touch” button and select “New Patient” or click the Contact tab, scroll to the bottom of the page, and fill out the form).

Prefer paper? Download a new patient form (found under the “Forms” tab) from the website, print it out and bring it with you. Completing this form in advance saves time and helps Dr. Kyle prepare for your consultation. Just be sure to fill out the form that best refers to your situation (either the self pay/health insurance new patient form or the motor vehicle accident new patient form).

Coming prepared goes a long way for your first appointment. Please arrive with the following information:

• Your completed new patient form (if you didn’t submit it online)
• A list of any medications you're currently taking
• A clear idea of your injury or pain and when it started
• Any questions or concerns you want to address
• Your goals — what do you hope to accomplish?
• Your insurance card (if paying that way)
